Home Health RN

Our home health care agency is rapidly expanding and we are hiring a Home Health Nurse effective immediately. Most of our patients are elderly so we hire the most compassionate and thoughtful talent in the field. We are looking for Registered Nurses in the Lake and Porter Counties. Registered Nurse will: Complete an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates patient nursing needs. Initiate the plan of care and makes necessary revisions as patient status and needs change. Use health assessment data to determine nursing diagnosis. Develop a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician. Counsel the patient and family in meeting nursing and related needs. Communicate with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required. Requirements: Graduate Degree from an accredited school of nursing. Current licensure in state for which care will be provided. Current CPR certification. 1 year of home health care experience preferred. Valid Drivers License with an automobile that is insured in accordance with organizational requirements and is in good working order.
