Registered Nurse (RN) Job Description
Duties and Responsibilities of Position: • Maintains the quality of client care/services in accordance with the agency’s objectives ad policies, through planning, coordination, and implementation and evaluation of the personal care provided. • Identifies and evaluates clients’ needs and condition during every visit and chart your observations • Perform evaluation tasks, including vital signs and medication review • Educate patients, personal care workers and their families on proper home health care strategies and procedures • Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient’s care plan • Provide palliative care as needed to keep the patient comfortable • Make recommendations for devices or tools that might improve the patient’s quality of life • Listen to the patient and respond to concerns or requests • Appraises PCW performance during their probationary period, 60 day supervisory and annually, provide the teaching and counseling necessary to enhance the delivery of services and the development of direct care staff. • Identifies and evaluates client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of • Develop the Plan of Care for a client after assessing the client and consulting the client and/or family member/responsible person giving full consideration to the client’s preferences for service arrangements and choice of personal care workers, interpret the pan to the personal care worker, include a copy of the plan in the client’s record and review the plan at least every 60 days and update it as necessary. • Participates in community related functions as necessary. • Promptly notify a client’s physician or other appropriate medical personnel and legal representative, if any, of any significant changes observed or reported in the client’s condition. • Completes client supervisory visits per policy. • Participates in client record reviews Job Qualifications • Must be a graduate of an accredited school of nursing. • Must be a Registered Nurse currently licensed under s441.06 Wisconsin Stats • Training/ experience in the provision o personal care services or a related program. • Must have at least one year of experience of home health nursing experience. • Must have a criminal background check. Job Conditions • Position is stressful in terms of meeting deadlines • May be required to bend, stoop, reach and move client weight up to 250 pounds lift and/or carry up to 30 pounds. • Travel is required to and from client’s home when supervision of care/services is scheduled. • The ability to communicate well, both verbally and in writing in English is required. • The ability to access clients’ homes which may not be routinely wheelchair accessible is required. • Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client’s condition and to perform client care/services Equipment Operation • Use of multi- line telephone, calculator and medical nursing equipment, i.e., glucometer, etc.
