Responsibilities: -Perform initial client assessments and develop and initiate the plan of care. Initiates the plan of care and makes necessary revisions as patient's status and needs change. -Develops a care plan that establishes goals based on nursing diagnosis. Includes the patient and the family in the planning process. -Assume responsibility to coordinate patient care for assigned caseload. -Completes and submits accurate and relevant clinical notes regarding the patient's condition and caregiving. -Attends interdisciplinary meetings. -Coordinate interdisciplinary services and obtain physician orders for appropriate care. -Administer nursing education. -Communicate continuously with patients, families, and other care team members every 50-60 days. Registered Nurse - RN Case Manager Qualifications: -Licensed as a Registered Nurse / RN in the state you work in -A valid Driver's License and reliable vehicle with current insurance is required. -Excellent at planning, organizing, and communicating.
