Registered Nurse (RN) Job Description
Mountain View Healthcare Center, a leading provider of Skilled Nursing Services located in Santa Clara County, CA, is presently seeking an experienced full-time Registered Nurse (RN) to join our team for any shift; AM/PM/NOC. Responsibilities: The Registered Nurse will ensure that resident's needs are met in accordance with professional standards of practice through physician orders, facility policies and procedures, and federal state and local guidelines. Observes, evaluates and gathers data, confers with and is directed by the Director of Nursing to meet the emotional, physical and cognitive needs of the patients. • Conducts an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs within timeframe specified by facilities policy and procedures or protocols. Notifies MD of abnormal findings. • Conducts ongoing assessments as determined by the patient’s condition and/or the facility’s policies & procedures or protocols and prioritizes care accordingly. • Develops plan of care that is individualized for the patient reflecting collaboration with other members of the healthcare team • Performs appropriate treatments as ordered by physician in an accurate and timely manner. • Provides individualized patient/family education. • Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution. • Answers phone calls, assists public, and forwards appropriately if necessary in a professional and friendly manner. • Initiates emergency resuscitative measures according to appropriate resuscitation protocols. • Reports patient condition to appropriate personnel during each shift. • Adheres to standards of safety and infection control in delivery of patient care. • Assists nursing staff in other departments as requested and qualified when need arises. • Maintains current CPR and ACLS status. • Follows procedure for medication administration and narcotics control. Documents medications and nursing care in a timely and accurate manner. • Assures that consents are obtained when needed and that all records are signed and legally complete. • Supervises LVN’s and non-professional staff in delivery of care to assigned patients. Make work assignments and act as a resource person for less experienced staff. Supplementary Pay and Benefits: Extra $5 per hour for NOC shift Extra $2 per hour for PM shift Extra $3 per hour for Weekend shift Signing-on bonus available to qualifying candidate Minimum Qualifications Possession of a valid permanent/temporary (including interim permit) California Registered Nurse License. Experience as an RN in a skilled nursing facility, acute care setting, or in-patient rehabilitation or in-patient behavioral health setting, is preferred.
