Our dedicated Wound Care Coordinator specializes in assessing and treating patients with complex wounds, ostomies, or continence conditions. They work closely with members of interdisciplinary team to ensure that personalized treatment plans are followed correctly in order to prevent future infection or injury. At the same time able to participate in discharge planning, development and implementation of resident care plans and resident assessments, as necessary. Schedule : 1st Shift Requirements RN Compassionately motivated about serving and celebrating seniors Act with integrity by providing specialized care to residents with Alzheimer’s & Dementia Contribute to an inclusive culture thriving on teamwork and positive attitudes Promote innovation including operating an electronic medical record system WCC preferred Meets documentation requirements and maintains accurate records for MDS assessments and restorative nursing interventions Able to plan, develop, organize, implement, evaluate, and direct restorative care services, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern nursing care facilities. Able to participate in discharge planning, development and implementation of resident care plans and resident assessments, as necessary.
