Job description Care One Hospice & Palliative Care is currently seeking a (F/T, P/T, or Per-diem) Registered Nurse (RN) The position is for a rapidly growing hospice organization in Tarzana, which provides services in the greater Los Angeles County and surrounding areas. At Care One Hospice & Palliative Care, we are committed to maintaining the highest standard of care for our patients. Our goal is to exceed the expectations of our patients and their families. We are looking for compassionate nurses who understand the special challenges that arise in hospice with patients and their family and who are able to provide care and comfort in a home type setting or skilled nursing facility. Duties and Responsibilities: Develops, coordinates, and follows the patient's individualized plan of care Provide regular visits to re-assess the patient's needs and update the plan of care. Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care. Obtain data on physical, psychological, social and spiritual factors that may impact patient and family`s needs and initiate intervention by other members of the interdisciplinary team. Initiate communication with the attending physician, other interdisciplinary staff members for the purpose of coordinating optimal care. Effectively utilize resources to assist in meeting the patient's needs. Participates in patient care meetings as assigned Participates in weekly IDT meetings Appropriately delegates nursing tasks to Hospice Aides/Homemakers, Volunteers, and Licensed Practical/Vocational Nurses Performs and documents care adhering to the professional, hospice, regulatory body, and nursing standards. Completes documentation at the time of the event and at the location of care unless otherwise directed Uses equipment and supplies effectively and efficiently. Demonstrates an ongoing commitment to professional growth. Participates actively in meeting the goals and objectives of the hospice organization. Function as a team lead under the supervision of the Clinical Supervisor. Participates in the on-call rotation Qualifications One-year prior professional nursing experience preferred Graduated from an accredited Registered Nursing program Possesses a valid CPR certification Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Willingness to travel to patient locations. Demonstrates strong verbal and written communication, as well as organization skills. Home Health, or Hospice experience, a plus.
