Home Health and Hospice LVN

Matrix Home Health is an established growing home health care agency seeking Vocational Nurses (LVN) to make home health care home visits in the South Bay area. Cities in the South Bay Area include: · San Pedro · Rancho Palos Verdes · Torrance · Long Beach · Etc. Pay Rate · Compensation will be determined based on visit (Per-diem) Responsibilities As a LVN, your main task is to care for our clients and facilitate their recovery. You will be responsible for educating them and their families on disease prevention and the promotion of healthy habits. Additionally, observing the proper documentation of each visit and procedure is a must. The goal is to promote a client’s well-being by providing proper nursing care. Moreover, you must be able to perform the following duties and responsibilities with integrity and proficiency: Complying with company policies and procedures Following Medicare and state regulations (HIPAA, etc) Providing direct patient care, home visits and clinical assessments Work collaboratively with other members of the home health team Usage of electronic documentation Required Documents and Qualification Home Health Experience is preferred Computer Skills is a must Bilingual Spanish speaking is a plus Proof of working experience as a LVN (minimum of 1 year) Valid California LVN license Valid CPR Certification Valid California Driver License Proof of Auto Insurance Job Types: Full-time, Part-time, Contract Pay: $35.00 - $40.00 per hour
