Our growing home health agency is looking for a Registered Nurse (RN) to grow with our team. Responsibilities: Assessment and Care Planning Conduct comprehensive patient assessments to determine their healthcare needs. Collaborate with physicians and healthcare teams to develop individualized care plans. Continuously evaluate and update care plans to meet the changing needs of the patients. Medication Management: Administer medications and treatments as prescribed by the physician. Monitor and manage medication schedules, dosage, and side effects. Educate patients and their families on medication management and proper usage. Patient Monitoring Monitor and record vital signs, symptoms, and patient progress. Identify any changes in the patient’s condition and report them promptly. Provide skilled nursing care, wound care, and other medical procedures as needed. Education and Support Offer health education to patients and their families on disease management, nutrition, and preventive care. Provide emotional support and comfort to patients and their loved ones. Promote independence and self-care skills for patients. Documentation and Reporting Maintain accurate and detailed patient records and documentation. Regularly communicate with the healthcare team, including physicians and case managers. Ensure compliance with all regulatory and agency policies. Safety and Infection Control: Implement infection control practices to maintain a safe environment for patients. Adhere to safety protocols and procedures to prevent accidents and injuries. Qualifications: Registered Nurse (RN) with a current, active license. CPR and Basic Life Support (BLS) certification. Valid driver’s license required. Minimum of 2 years of experience in home health setting. Minimum of 5 years of nursing experience. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities. Compassion, empathy, and a commitment to patient-centered care. Reliable transportation for home visits. Ability to work independently and as part of a team.
