Communication:

Providing education to patients and families regarding medical conditions, treatments, and preventive measures.
Care Coordination:
Coordinating patient care across various healthcare settings (e.g., hospitals, clinics, home care).
Collaborating with social workers, case managers, and other support staff to ensure continuity of care.
Patient Advocacy:
Advocating for patients' rights and preferences.
