DO NOT SEND YOUR LOVED ONES HERE. Immediately upon entering this facility we tried to discharge my grandmother within 10 minutes. The facility had the strongest stench of feces and urine. She was in a shared room that shared a bathroom with 4 patients. In the 4 days we were there, it was never cleaned. Everyone is fed the exact same food despite whatever dietary restrictions they may have, and it was disgusting. She fell in the middle of the night and we were never informed. They said they took an X-ray of her arm, which they lied. We all were waiting for results, including the nurse, who found out no X-ray was taken. They came 6:00 that night to do the X-ray. She was given shots in her stomach, which we were never told about. When they gave her a shower, her neighbor also went after my grandmother and reported that there was also feces on the floor. We immediately went into Omar’s office to complain 10 minutes after arriving. He was nice and said he really didn’t smell it but would have someone deodorize. My mother rented a transport vehicle and took her out before more harm could be done. One good thing is day shift personnel was very nice. BEWARE!!!!!
Just found out they had the cops "dump" my family member at the hospital emergency room, because he's not easy to care for. Update 12/2/25. My family member had been staying here for over 2 weeks, this is his residence, his belongings are there and from what I was told when the hospital sent him back the staff blocked the entrance, please anyone out there that might have family or loved ones here, please check on them
The staff is great. Even though I didn’t participate in many activities, they always had a lot of great activities for all the residents. The therapy department always pushed me as much as myself to gain my strength back. I made great friends with the other residences, and staff members. I’ll return as a visitor.
I had a relative stay at the Ladera facility. The medical staff were amiable and professional, but my family had an issue with the business operations manager. When my relative's insurance stopped covering the cost of his/her stay and he/she had to pay out-of-pocket, the business operations manager used high pressure tactics to try to demand details about my relative's personal finances from my family. My relative transferred to the hospital for treatment. When it came time for him/her to return to Ladera, the business operations manager advised my family that Ladera couldn't receive him/her because they could not provide the care that he/she needed. 1.) Ladera staff cared for my relative prior to his/her transfer to the hospital; I'm highly dubious that Ladera lost its ability to care for him/her during their treatment in the hospital. 2.) Ladera staff did not inform my family that they couldn't care for my relative until after we contacted them about transferring him/her back to Ladera. 3.) Ladera staff reportedly told the hospital social worker(s) that my relative did not pay his/her bill when I ensured it got paid. I don't know what is going on with Ladera, but I will not recommend it until I know there is an improvement.
The staff was sweet and kind. They were on time with my meds and therapy helped me walk around to gain my strength. The CNA down the north back hall always responded quick with assistance. the therapy dept is very knowledgeable and always found a way to work around my pain their personalities helped motivate my progress.
