Community Care is a Skilled Nursing Facility with 204 beds and multiple levels of care, including a large behavioral health patient population (ages 21 and up) within the center. • We are located near W 43 ST & State Street, just off 90/94 and a few blocks from the train station. I was pleasantly greeted at the front desk by the receptionist before I toured for a family member . I try not to alert the facility before I've been touring, I walked into diligent housekeeping in well lit floors. The nurses are very knowledgeable , as well as social service director . All my questions were answered and I'm confident in choosing this place for my loved one

Since reopening, Community Care Center has greatly improved. The environment is clean and orderly. The staff is great. The goal of this facility is to provide excellent care for the residents, and they are on track to meeting and exceeding this goal. I've been living there for 6 weeks. Indeed there is an abundant room for improvement. I've recently moved into a new room, which is not fully furnished. One of which is a lamp. Certain members of the staff are very unprofessional. I have delt with some who just cannot do a simple task from getting clean towels to installing a lamp. Certain needs here are not considered important. But it is important to me and other residents. Whoever reading this, you know who you are. And for those who agree. Will fully undertstand. And make their voices heard. For something that I asked for, 2 days ago! Broght me to a point of desperation, to get my point across on this site. Letting my voice be heard!
