My father recently completed a six-week physical and occupational rehab stay at Taylor Care Center after being hospitalized for a month. During his stays across two facilities, he was not provided any significant PT/OT. Because he was bedridden the entire time, he lost the ability to sit up on his own, let alone stand and complete basic tasks he was able to before his hospitalization. The entire team at Taylor Care Center was wonderful, friendly, and genuinely interested in caring for and improving my father from the day he entered to the day he left. Angeline, Verna, Rose, and Sandy (the nurses) were always so helpful, and I knew that even when we weren't there visiting, they were treating him with dignity and respect while ensuring he was being cared for. They always spoke to us every time we were there (which was essentially daily from anywhere to 1 to 2 hours) and updated us on my father's daily wanderings and jokes. The PT/OT team did a great job improving my father's strength and confidence to regain his mobility. Before starting at TCC, he would lament that "he was old and this is what his life will be like", but they didn't let him accept that. In those 6 weeks, he recovered to the point of being able to stand and use a walker to regain his mobility. We worried about him discharging before he was ready, but they got him ready. I don't think he would've ever recovered had he only had at-home PT/OT. Having daily, dedicated PT/OT was critical. The resident-to-nursing levels and PT/OT levels on the Medicare.gov site made me feel comfortable going with Taylor Care Center. While I was initially hesitant because it was an older facility (but very clean), it was the team at TCC that made us feel like we made the right decision at the end of the day. Thank you to the entire team at Taylor Care Center. You are spectacular!!!
Keep a close eye on your loved ones here. I question their staff and facility after 2 people I know passed extremely quickly after being admitted to their care. I didn’t know how to review this but based off my last visit before my mother in law passed. They wheeled her out in a non responsive state to us like it was normal and when we questioned why she was like this after a week prior being coherent and communicate they said this is how she was, or that’s not my patient it’s another nurses. The staff needs to be retrained on taking care of patients.
I appreciate the acknowledgement and attention I received to hearing me in my time of need by the entire program staff. I made the decision to make the best of my situation with determination and it was received with open arms.
I have been in Healthcare for 17 years. Taylor Care Center is absolutely amazing! The Administrator, DON, Unit managers, Wound Care, Dietary, Therapy and Housekeeping. I have met so many of the employees here. This is how a Healthcare facility should be ran. I highly recommend Taylor Care Center!
Taylor Care skilled nursing is 6 stars!! If you have ever visited skilled nursing care centers, you immediately get a vibe and can literally smell if the facility is well staffed and managed. Taylor is by far one of the best in Jacksonville and maybe in the region/state. The nurses and CNAs are informed, knowledgeable, kind and professional! I feel extremely lucky to have gotten a spot here for my father.
