A Medicare-certified skilled nursing facility provides 24-hour nursing care, rehabilitation services, and assistance with activities of daily living for patients who need skilled nursing or rehabilitation services on a daily basis.

This facility is part of a Continuing Care Retirement Community (CCRC), which offers a continuum of care on a single campus—from independent living to assisted living to skilled nursing care. Skilled nursing facilities provide 24-hour nursing care for patients who need rehabilitation services after a hospital stay or ongoing care for chronic conditions. Services include skilled nursing, physical therapy, occupational therapy, speech therapy, and assistance with daily activities.

Staffing Levels: Based on CMS payroll data, this facility provides approximately 5.18 hours of total nursing care per resident per day (2.52 hours from CNAs, 1.22 hours from LPNs, 1.44 hours from RNs). Physical therapy staffing is 0.2 hours per resident per day. This represents above-average staffing compared to national benchmarks.

Medicare Quality Ratings: According to CMS, The Terraces of Phoenix has an overall quality rating of 5 out of 5 stars, which is much above average compared to other nursing homes nationwide. Individual category ratings are: health inspection: 3 stars, staffing: 5 stars, quality measures: 5 stars.

Medicare Coverage: Medicare covers skilled nursing facility care for up to 100 days following a qualifying hospital stay of at least 3 days. Days 1-20 are fully covered by Medicare, days 21-100 require a daily coinsurance payment. Many residents also use Medicaid, private insurance, or pay privately for long-term stays.

Questions to Ask: When visiting a nursing home, ask about: staff-to-resident ratios and RN coverage around the clock, how they handle medical emergencies, activities and therapy programs, how they communicate with families, their approach to falls prevention, and policies for managing resident complaints.

I would like to share my positive experience at that facility. My 90 y.o mother Zoua, was there overnight, but decided to go back home. Staff was excellent and very friendly. There was an RN Jennifer, who took a great care of my mom. Very kind and caring. Jacqueline Elam RN was extremely helpful in answering all of our questions in a great detail. She spent almost 30 min with us and was very courteous and professional. Both assisted my mom when she decided to go home. Both were very supportive and understanding. Great place. Great staff. Very friendly and caring atmosphere. Thank you.

My husband was there for 2 months. He would not have walked out without the care and prayers from everyone. The physical therapists, nurses, aides, management, dining personnel, too many to mention. The food was great with so many options! The rooms were cleaned and well maintained. The grounds are amazing and very well kept. Beautiful flowers everywhere with our beautiful native desert plants throughout. We would highly recommend for anyone needing special care or even a new retirement community.

My experience working with a private client at The Terraces of Phoenix (Assisted living area), is superior, the entire team that assisted in the care of the resident, from front desk clerk delivering her package to apartment, Med techs, house keeping, most of all the amazing kitchen/ dinning area, the healthy and delicious meals from the chefs, at times goes above and beyond their duties ensuring the resident are happy and well fed at meal times, fine dinning styles with traditional and international menu idea. Beautiful landscape, walking trail with wonderful sight seeing for resident. Not to mention activities daily. If you are looking for a place to called home, peace of mind. Denfinely recommend this place. Five Star +!!

The people that work there are nice and are trying their best, but they are severely understaffed. Which means that the patient suffers. When someone needs assistance to go to the bathroom but it take 20 mins-30 mins to get help. Well you can imagine how the patient felt holding it in. Anyways the area is nice which is a plus.

Do not send your loved one here for rehab. No consistency or communication between front line staff. Some good front line nurses and aids but generally no one seemed to know anything about my loved one and would ask us questions about his care. Nurse call button left in a drawer out of reach and used biohazard medical equipment left in the bathroom sink. Admin even worse. My loved one was there for over a week and we had to ask for communication with the SW and other support professionals. Requests for calls from admin delayed by two days. Health doesn’t take the weekends off. Case planning meeting sounded like a cut and paste of every other patient and like they never even met my loved one who was declining under their eyes. Do not let your loved one be sent here for rehab.
