Once individual needs are properly assessed, our staff and caregivers work to accomplish the following goal
• Provide quality and reliable care in our client’s homes and minimize the impact of an illness or disability
while maximizing independence and restoring, maintaining and promoting health
• Facilitate a collaborative communication process between the client, family, medical professionals, and
caregiving staff
• Respond efficiently to our client’s immediate needs, including hospital and facility discharges, respite care,
and emergency coverage.
