Caregiver Job Description
ABOUT THE ROLE
The Resident Care Coordinator plays a key role in supporting the physical, emotional, and spiritual wellbeing of our residents. This part-time position blends wellness assessments, clinical communication, staff coaching, and documentation review. The Coordinator promotes high-quality care by maintaining accurate MARs and ADLs, conducting regular resident assessments, ensuring care plans are implemented correctly, and guiding staff through best practices.
This role is ideal for someone who enjoys working closely with seniors, collaborates effectively with caregivers and clinicians, and embraces a faith-based approach to whole-person care.
CORE RESPONSIBILITiES
Resident Wellness & Assessments
Oversight of resident health through weekly checks, monthly assessments, monitoring, and making recommendations that guide care.
Conduct weekly wellness checks for all residents, including vitals (BP, pulse, temperature, respirations as needed, oxygen saturation, blood sugar when applicable).
Assess skin integrity, wound care needs, mobility changes, and fall-risk indicators.
Complete monthly Memory Care, Dementia, and Quality of Life assessments documenting changes in mood, appetite, sleep, behavior, and overall condition.
Provide recommendations based on findings and document all assessments accurately.
Support staff in understanding and implementing care changes.
CLINICAL CARE COORDINATION
Communication bridge between clinicians, residents, and Resident Caregiving staff to ensure care recommendations are clear and implemented.
Liaise with visiting nurses, physicians, PT/OT providers, and other clinical partners.
Collect updates on care recommendations and communicate them internally.
Attend resident medical appointments when possible to clarify concerns and understand care plans.
Translate clinical instructions into practical steps Resident Caregivers can follow.
Track therapy recommendations, follow-up orders, and medication/treatment changes (not dispensing).
SUPPORT, TRAINING & COMMUNICATION
Guiding, coaching, and supporting caregivers to deliver high-quality, compliant care.
Provide coaching and corrective guidance on Medication administration, Personal care and ADLs, etc.
Respond to staff questions in Slack regarding medications and resident care.
DOCUMENTATION AND ADMINISTRATIVE SUPPORT
Ensures accuracy, compliance, and continuity in resident records and internal communication.
Maintain accurate MARs and ADL documentation for all residents.
Identify missing or inconsistent documentation and follow up with staff to correct it.
Record assessments, observations, and care recommendations.
Update charts with physician orders, medication changes, and treatment adjustments.
Keep resident health files organized and up to date.
QUALIFICATIONS
Strong experience in caregiving, medication management, and care documentation.
Experience with resident care, ADLs, and MAR documentation.
Ability to recognize changes in resident condition and make appropriate recommendations.
Experience with vitals, wound care basics, and general assessments.
Strong communication skills.
Ability to professionally redirect and mentor staff.
Comfortable working independently without supervision

COMPENSATION AND SCHEDULE
Compensation: $20+/hour
Weekly Schedule: 12–15 hours per week (semi-flexible) with possible extra hours filling in for caregivers, if desired.
Required:
Monday Administrative Meeting (1–1.5 hours between 11am)
Friday Caregiving Support (7:00–9:00am)
The remaining weekly hours are flexible.
Required: 3–4 scheduled workdays per week
