We are seeking a dedicated and compassionate Care Coordinator to join our team at Prestige Home Care Angels, Inc. As a Care Coordinator, you will be responsible for managing the intake process for clients seeking in-home care services. Answer phone calls, patient documentation inputting notes in the portal. The applicant needs to be organized, pays attention to detail, team player and has general office knowledge. Position: Sundays to Thursdays from 9am to 5pm after Training Completed Essential Duties and Responsibilities The essential functions include, but are not limited to the following: · Answers phones, receives all intake information for paraprofessional cases and enters the appropriate information in the computer. · Completes intake forms and routes them appropriately for admission approval. · Assists in the scheduling of appropriately skilled person to fill the case, inputs scheduling information on the computer and continues to assist in the coordination of the scheduling. · Schedules replacement workers as needed and inputs scheduling information. · Assists in communication with clients/families on employee related issues. · Handles matters related to staff and the client we serve, with the exception of issues that require managerial expertise/clinical judgment. · Assists in the maintenance of case management notes related to personal problems and/other situations that arise with staff including those relating to client care/service. · Communicates on a routine basis with contract agencies to maintain smooth relations and to obtain needed information. · Documents and reports changes in client status as well as any client complaints/grievances to upper management team · Documents and reports to upper management any unsatisfactory performance from home care aides. · Performs other duties as directed. Qualifications: -General office knowledge -Excellent communication, problem-solving, and analytical skills -Knowledge of managed care approach to health care delivery -Should have experience with a variety of office software (email tools, spreadsheets and databases) and be able to accurately handle administrative duties. Computer proficiency with Microsoft Office Programs (Word, Excel) -Must have the ability to perform in a team environment -Strong organizational skills with ability to work independently Requirements: - Should be able to work required schedule - Must be registered with the State of California as a Home Care Aide. If not registered, we can help you with the registration process - Must have at least 3-year hands on experience working with elderly, mentally disabled and autism individual -Must have at least 1-year receptionist experience - Must have TB or chest x-ray test result completed within last 6 months - Excellent written and verbal communications skills -Worked with the Regional Center is a plus! -On occasion, may need to go out in field for staffing fill-in coverage If you're passionate about making a difference in people's lives and have the qualifications we're looking for, we'd love to hear from you!
