Care Professional Job Description
Title: Care Professional Reports To: Director of Operations
Position Purpose: To provide direct care to clients to help each client reach and maintain their highest level of performance, safety, and independence.
Principal Responsibilities
•
Plan of Care & Care Delivery
Understand and adhere to each client’s individualized Plan of Care, assist with periodic evaluations as directed, and provide permitted hands-on care including personal care, toileting, mobility assistance, exercise, range of motion (ROM), ambulation, and the application of hot/cold therapies, support stockings, and ace bandages.

Personal Care & Daily Living Support
Assist clients with activities of daily living including bathing, skin care, dressing, grooming, feeding, oral hygiene, bedtime care, and communication needs, while maintaining dignity and respect.

Medication Support
Provide medication reminders only, as outlined in the Plan of Care; Care Professionals do not administer medications.

Safety & Environment
Maintain a clean, safe, comfortable, and healthy client environment; follow all safety precautions and promptly report hazards, unsafe behaviors, or environmental concerns to a supervisor.

Observation, Reporting & Documentation
Observe and accurately document client status, care provided, and services rendered; report changes in condition on the same day of observation and report all incidents or accidents immediately.

Professional Conduct & Boundaries
Engage in respectful social interaction while maintaining professional boundaries; demonstrate professionalism, confidentiality, and respect toward clients, families, and coworkers.

Attendance & Policy Compliance
Maintain regular and reliable attendance to meet client care needs; adhere to all SYNERGY HomeCare policies and procedures and perform other duties as assigned.

Support of fellow co-workers and clients
Expected to have other care professionals shadow you at client’s homes to ensure smooth transitions and gather knowledge and information by someone who has already worked with that client.
Position Specifications
Working Conditions: May include exposure to infectious materials, body fluids, medical equipment, environmental hazards, and travel to client locations.
Hours of Work: Hours vary based on client needs and Care Professional availability; no guarantee of hours.
Other Requirements: Must be in good physical condition, have a current negative TB test as required by Minnesota law, and be free of communicable diseases. Any past or current injury affecting physical activity must be disclosed prior to orientation. Must have a valid driver’s license with proof of auto insurance or reliable transportation. Prior in-home care experience preferred but not required.
Education & Training Requirements
• Must be at least 18 years of age • High school diploma or equivalent preferred • Current Adult and Infant First Aid and CPR certification required • Completion of all Minnesota-required Home Care training (Basic, Comprehensive, and/or Memory Care) • PCA, HHA, CNA certification required for personal care, or completion of SYNERGY RN skills training and sign-off by RN

Ability to safely perform personal care and mobility assistance.

Ability to remain calm, flexible, work independently, follow care plans accurately and be professional at all times in varying situations.

Physical Requirements
This position may require standing, walking, bending, lifting, reaching, and repetitive motion. Ability to lift, carry, push, or pull up to 25 pounds may be required.
Cognitive Requirements
This position requires reading comprehension, problem-solving, confidentiality, verbal and written communication, multitasking, and the ability to manage interruptions.
