Certified Nursing Assistant (CNA) Job Description
Primary Function: Responsible for providing personal care and household services to tenants/clients. To communicate to the interdisciplinary team about pertinent information about tenants/clients. Responsible To: Reports to Program Nurses. Responsible For: Working in accordance with the written approved service plan in a professional, respectful, and confidential manner. Major Functions: 1. Gives or assists with personal care services as outlined in the service plan of GAFC tenants. 2. Provides household services. 3. Observes records and reports pertinent information to team regarding tenants. 4. Promotes and encourages independence in tenants. 5. Records in daily personal care log. Duties and Responsibilities: A. Personal Care Services 1. Performs care daily in a safe, non-threatening, unhurried, competent manner. 2. Assists with bathing as outlined in service plan – bed, tub, sponge, or shower (not complete baths, unless authorized on case-by-case basis) 3. Care of hair – combing, brushing, shampooing 4. Skin care – Observing for bruises, scraps, cuts, redness etc and report to nurse. 5. Care of mouth, teeth, and dentures – cueing for brushing, removing, and soaking of dentures, rinsing mouth. Teach tenants the rationale for good oral care. 6. Dressing and undressing where necessary, as well as, cueing for changing into clean clothes and appropriate clothing for weather conditions. 7. Feeding, or cueing to eat, meal preparation and supervision of consumption in accordance with an individual service plan. 8. Assist with toileting and incontinence care as appropriate 9. Assist with cueing to use assistive devices when necessary as outlined in the service plan for the tenant. 10. Assist with transfers from bed to chair, chair to chair, chair to bed, etc. for residents who can brace at least 50% of their own weight and is outlined in the tenant’s service plan. 11. Practices essential principles of universal precautions and infection control 12. Cue residents to take medication if outlined in individual service plan. 13. Attends service training sessions when requested. 14. Attends scheduled supervisions with supervisor. 15. Reports any pertinent changes in tenant’s health status to RN/SW. 16. Attend and assist with educational activities per supervisor’s request. 17. Ruggles Assisted Living employees must maintain 11 hours annually of EOEA Required in-service hours
