DUTIES
To provide nursing care in accordance with the patient's plan of care to include comprehensive health and psychosocial evaluation, monitoring of the patient's condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.
RESPONSIBILITIES
Conduct initial and ongoing comprehensive assessments of patient's need, including Outcome and Assessment Information Set (OASIS) and reassess at appropriate time points,
Supervise LPNs and paraprofessionals providing services to patient.
Act as a liaison with physicians, hospitals and specialty organizations and provide them with information and instructions
Prepare clinical and progress notes; evaluates the effectiveness of outcomes of care.
QUALIFICATIONS
Must be licensed as a Registered Nurse in the state of Massachusetts
Bachelor's degree preferred
Must have a valid drivers license and a reliable means of transportation
Must have CPR and HIV courses.
Must be willing to undergo a criminal background check.
