Our Rehab Unit focuses on rehab to home. You will work with a team that includes nursing, respiratory and therapy in a supportive environment. We are the bridge on a patient’s journey between the hospital and recovery. We provide that critical transition to optimal independence. As a Certified Nursing Assistant, you will: 1. Perform resident care according to Policies and Procedures, regulatory guidelines, and within scope of practice to enable resident to reach highest level of function possible. 2. Assist resident with activities of daily living such as personal hygiene and grooming, mobility, dressing and undressing, bowel and bladder functions, eating and drinking, repositioning etc. 3. Perform additional daily/weekly duties assigned such as daily weights, supply restocking, etc. 4. Observe and report changes in the resident’s condition to the Licensed Nurse such as alteration in vital signs, skin breakdown, weight loss or gain, injuries or incidents etc. 5. Utilize the EMR to complete the CNA Point of Care documentation each shift. 6. Initiate emergency procedures, if needed. 7. Prepare resident’s room for admission, obtaining equipment and supplies as needed. 8. Prepare resident for discharge. Remove equipment from room for terminal cleaning. 9. Follow safety policy and procedures including those for fire and disaster management. 10. Report any hazardous conditions, equipment, accidents and incidents to supervisor. 11. Use equipment correctly and safely. 12. Be responsible, when needed, for resident activity programs, including off-site outings. 13. Assist the therapy staff as needed. 14. Participate in Performance Improvement Program. 15. Actively participate in Infection Prevention Program following any isolation procedures that may be required. Requirements for this position Education: · High School diploma or G.E.D. · Completion of a CNA certificate program. Experience: · One year CNA experience desired. Certificates, Licenses, and Registrations: · Current CNA certification issued by this state. · Current Basic Life Support (BLS) Certification for Health Care Providers. Skills: · Able to effectively communicate (verbally and in writing) with all staff, residents, and their families and/or authorized representative. · Able to add, subtract and multiply. · Able to problem solve, plan and organize within scope of the position. · Able to work with and relate to behaviorally challenged residents.
