Licensed Practical Nurse (LPN) Job Description

We are the bridge on a patient’s journey between the hospital and recovery. We provide that critical transition to optimal independence. As a valued member of our HealthBridge team, you will: 1. Direct care to team members 2. Act as a patient liaison and advocate. 3. Assure proper documentation utilizing the EMR including documenting assessments, initiating care plans, formulating, updating, and revising treatment plans and goals. 4. Participate in discharge planning including identifying home educational needs and instructing and educating patients for ADLs. 5. Assure orders are processed including checking and signing off orders, accepting telephone orders from physicians and ensuring documentation of advanced directives. 6. Make initial and continuous patient assessment including communicating the interpretation of lab data, EKG’s, and other tests. Consult with physician manage patient’s in-house transfer through system, review drug therapy and coordinate administration of treatments between disciplines. 7. Coordinate all aspects of patient care, including planning, intervention, education, and evaluation including age-specific knowledge, phlebotomy, IV access and staple removal. 8. Initiate physician orders, participate in Code Blue response, and provide patient education. 9. Ensure patient handoff to other disciplines through verbal or written communication. 11. Assist in providing adequate staffing and participate in policy development. 12. Function as a charge nurse, assigning care of patients to caregivers according to policy. 13. Participate in education and staff development through direct learning, teaching, orientation and peer review. 14. Perform duties and responsibilities in a manner consistent with our mission and values. Education: Current licensure as a LPN or RN in the state of Missouri required. BLS Certification
