Registered Nurse (RN) Job Description
Summary: Provides and assesses health and needs of residents. RN will also work closely with administrative and medical staff to keep resident records up to date and handle resident assessments. Works as a member of and collaboratively with the interdisciplinary team with a focus on maintaining the residents’ care. Scope of Practice: Practice within the scope of certification and by state and federal regulations and in accordance with the standards as defined in the currently recognized state RN Guidelines. Practice in accordance with Aperion’s clinical and administrative policies and procedures. Responsibilities: 1. Provides medical assessments, in accordance with their scope of practice (RN), to the residents to meet their daily functions. 2. Direct and supervise LPN, CMT, and C.N.A staff. 3. Tasked in completing various resident’s paper work; including the monitoring, charting and reporting which will meet with the current guidelines and regulations as these regulations may vary each year. 4. Assess how residents with skilled nursing along with long term healthcare function, and take the necessary steps needed to support them. 5. Collaborate work with other medical staff and run assessment care plan meetings. 6. Responsible in ordering, interpreting and evaluating diagnostic tests to identify and assess resident’s condition. 7. Modify resident treatment plans as indicated by their responses and conditions under supervision of doctors or DON instructions. 8. Monitor all aspects of resident’s care, including diet and physical activity. 9. Respects privacy, follows HIPPA requirements, only shares information on an as needed basis and always knocks on a resident’s door before entering. Discusses concerns about a resident in private with appropriate people, for example – the leadership team, resident’s family, etc. 12. Thinks of new ideas and fun activities that will interest the residents. 13. Is sensitive to residents with dementia and tried to find ways to connect with them. 14. Encourages and supports residents to continue to maintain independence and to continually learn and grow. Qualifications, Knowledge, Skills & Abilities: 1. A desire to be part of the “Person Cent Centered Care” team. 2. Current RN License. 3. Background check with no disqualifying offenses. 4. Experience with direct care preferable.
