Certified Nursing Assistant/Personal Care Worker Professional Role: A critically important role in the daily life of this community is filled by the certified nursing assistant and personal care worker. You are the person who knows the neighbors most closely and often times their closest friend. You provide medical and physical care to the neighbors to meet their daily needs and enable them to function at the highest possible level. Eliminating the plagues of loneliness, helplessness, and boredom motivates you to work with the whole care partner team to identify ways to deepen relationships, create meaning, and aid neighbors in being the decision makers in their lives. As part of the neighborhood team you help to support the activities of the neighborhood including recreation, restorative exercises, housekeeping, and meal service. All interactions with neighbors, co-workers, family and visitors is performed in a kind and caring manner. You are part of a care partner team responsible for creating home for the neighbors here in our community. Duties & Responsibilities of this Position: Certified Nursing Assistant/Personal Care Worker: You attend to all neighbor personal hygiene & personal care needs, including but not limited to grooming, hygiene, dining & nutritional, mobility, elimination, psychosocial & safety needs. You organize all supplies necessary to complete the tasks. You support neighbors in self- care, & provide care as necessary for those unable to care for their own needs. You follow universal precautions, proper infection control, sanitation & safety standards of practice in all work & activities. Meeting your continued education needs on an annual basis Updating ISP as needed Housekeeping – dusting, vacuuming, cleaning bathrooms, deep cleaning of apartments, etc. You provide restorative therapy under the leadership of the Resident Care Coordinator/therapy professionals & in accordance with the neighbors functional maintenance plan You distribute medications in accordance with physician orders You observe neighbors closely, identifying changing needs & conditions at first indication. You notify the attending physician in a neighbor’s change of condition and Care Coordinator or RN. Process physician orders and communicate new orders to pharmacy You are responsible for weekly vitals & to update MD with changes You perform laundry duties for each neighbor & as needed You participate in the neighbor’s care planning & care conference, serving as the neighbor’s closest advocate in directing their care & honoring their preferences You keep apartments clean and tidy You keep wheelchairs/walkers clean You know and comply with neighbor rights and promptly report neighbor complaints, grievances or violation of neighbor rights Dining: You participate with the household team to make the dining experience pleasant You follow safe handling & sanitation practices You participate in serving meals You participate in temping food as it cools down Basic Core Competencies: Homebuilding: You respect the neighbor’s apartment as home, always knocking or asking permission to enter. You foster interaction between neighbors. You contribute to making the household a happy place, working with the household team to get things done. You see the possibilities of creating a daily life that naturally facilitates pleasurable moments, laughter & fun with meaningful relationship building. Person-Centered: As a care partner you must develop (or possess) an innate ability to observe & connect with neighbors in a way that gives them clear insights into each individual’s preferences & concerns. You call neighbors by desired name. You stand or crouch to be face to face. You have full attention on & converse with the neighbor when assisting with personal care or while dining. You push wheelchairs slowly and you ask neighbors opinion in daily decision making. You approach neighbors in a calm, gentle manner. You ask the neighbor when he
