The Care Coordinator is a key member of our care team, delivering high-quality home care services by connecting clients, families, and caregivers. In this hands-on role, you will support quality assurance, participate in client consultations, design personalized care plans, and help mentor and train our caregiving staff. Working alongside leadership, you’ll balance evolving client needs with caregiver guidance and office support—ensuring every interaction reflects a caring, dignified, and professional standard. This person must have a current CNA certification and five year experience.
