RN Case Manager - Hospice

Envision Home Health and Hospice is seeking a full-time RN Case Manager who is passionate about providing quality care to patients and families in various residential settings including homes, assisted living facilities, and skilled nursing facilities within Pierce County, WA, (Tacoma, Kent, Renton, Auburn, Lakewood, Lacey, Puyallup or surrounding areas). ***$15,000 Sign On Bonus*** RN Case Manager responsibilities include: Coordinating all aspects of the interdisciplinary care and services provided to hospice patients and their families. Provides education to families and caregivers Develops, implements and reviews healthcare plans Advocating for patients and their families Documentation of care Nursing assessments Symptom management Promoting comfort and quality of life. Qualifications: Current RN license to practice in the State of WA. Current WA driver’s license Reliable transportation Maintain current CPR certification Excellent communication skills Minimum of two years nursing experience with at least one year in hospice or home health preferred, but not required. Self-directed and able to work with minimal supervision. Demonstrate excellent observation, problem solving, verbal and written communication skills, nursing skills per competency checklist. Shows ability to organize and prioritize workload independently.
