Licensed Practical Nurse (LPN) Job Description
OVERVIEW: Assist Unit Coordinator in the responsibilities for the total patient care within the Unit in which assigned, including supervision of assigned CNA’s. Perform any or all professional nursing duties. May be asked to act as Unit Coordinator of the Unit and at that time will follow responsibilities listed for Unit Coordinator. May respond to emergencies in the Residents apartments. Blood and body fluids exposure: Category I. RESPONSIBILITIES: Include the following. Other duties may be assigned. • Keeps confidential all information not specifically authorized to be given to others. • Takes and records Resident's vital signs. • Dresses wounds, gives enemas, douches, alcohol rubs, and massages. • Applies compresses, ice bags, and hot water bottles. • Observes Residents and report adverse reactions to medication or treatment to medical Staff in charge. • Administers specified medications as ordered, and notes time and amount on Residents’ charts. • Assembles and uses such equipment as catheters, tracheotomy tubes, oxygen supplier, CPM machine, tube feedings, and IV’s. • Collects samples, such as urine, blood, and sputum, from Residents for testing, as ordered. • Prepares or examines food trays for prescribed diet and feeds Residents as necessary. • Records food and fluid intake and output. • Bathes, dresses, and assists Residents in ambulation, turning, and toileting. • Cleans rooms, makes beds, and answers Resident's calls. • Provides post mortem care to deceased Residents. • Inventories, requisitions supplies, and restocking of supplies. • Supervises and/or performs and assists with quality patient care according to outlined policies and procedures for Residents total needs, including disciplinary actions when necessary. • Maintains good working relationships and teamwork between all departments and co-workers. • Maintains a pleasant, warm, friendly, and caring communication with Residents’ family, friends, visitors, and responsible parties. • Effectively assigns duties to nonprofessional Staff according to Residents’ needs. • Maintains records of nursing and medical treatments and related services executed by Nurses. • Makes periodic rounds to observe and evaluate the Residents and to see that quality care is being given. • Assists in implementation of policies and procedures and follows up to determine compliance. • In cooperation with ADON/DON reviews overall disaster and fire plans to determine that each Team Members knows, understands, and practices his/her own role in the event such plans are executed. • Implements and updates as necessary individual Resident care plans. • Sees that no variation exists because of a Resident’s race, color, creed, national origin, religion, or social position. • Organizes and directs daily nursing conferences for shift Staff. • May administer and chart medications as ordered. • Accepts telephone orders from physicians. • Administers and carries out all treatments as prescribed by a physician. • Responsible for all admission and discharge duties as they relate to Nursing Services. • May accept Residents’ personal property for safekeeping, forwarding to Administration Office. • Notifies attending physician and family if necessary of any unusual condition of the Residents. • Assists Resident's to bathroom and offer bedpans and urinals as necessary. • Assists patients with such basic needs as dressing, eating, and bathing and encouraging patients to do things for themselves to retain feelings of independence and self-esteem. • Must maintain current CPR Certificate within six months of employment. • Observes facility policies and procedures regarding attendance, timeliness, rules, teamwork, customer service, and other policies/procedures that may be introduced from time to time. • Provides Resident monies after hours per policy. • Observes and follows safety policies and procedures. • Observes and follows infection control policies and procedures.
