This includes assessing client status and needs, developing individualized care plans, and providing skilled interventions aimed at achieving realistic client outcomes. The RN practices nursing with respect for individual, cultural and spiritual differences. Additionally, the RN collaborates and communicates with all members of the health care team to identify and create a holistic approach to care. **This is a part-time position that has the potential of up to 25 hours per week** About You: A minimum of 1-year nursing experience working in a hospital, skilled nursing facility, home health care or assisted living setting. Current license as a Registered Nurse in the State of Minnesota. Knowledge of home health care/assisted living requirements, regulations, and protocols a plus. Passion and commitment to clinical excellence, quality, and integrity Excellent interpersonal communication and patient & family engagement skills Effective verbal and written communication, assessment, care planning, teaching, clinical, and disease management skills. Knowledge of nursing care required by older adults and persons with chronic/acute conditions. Ability to work independently and demonstrate leadership skills for directing care activities of LPN/unlicensed professional staff as well as working well within a team environment. Effective organizational, critical thinking, problem solving, communication, and interpersonal skills. Familiar with health care programs, and community resources. Basic computer skills and ability to use electronic health record system. Knowledge and comfort level working with technology and agency support systems and tools Ability to comply with privacy and HIPAA laws. Flexibility with diverse clients, cultures, and environments Promotes the agency through positive representation and communication of its services Must pass a background study with the Minnesota Department of Human Services and negative Tuberculosis (TB) test. Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes.
