We are a Medicare Home Health Agency in need of Oasis experienced RN's to service the Palm Beach area. We are offering per diem positions which allows for a more flexible schedule. Requirements; Current state license as a Registered Nurse Minimum 2 years nursing experience Minimum 1 year Medicare Oasis Home Health experience Kinnser EMR system experience preferred Current PPD or Chest Xray Responsibilities: Complete initial assessment Initiate and develop plan of care Provide health care instructions to the patient Identify discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Update & communicate with the PCP / CM / ALF when necessary or condition changes.
