ACHC accredited and Medicare certified Home Health Care Agency need Home Health Aide for providing services to patients in home care setting in Duval and St. Johns counties. Responsibilities include routine visits, follow physician orders, communicate with patient care coordinator, follow Medicare & ACHC guidelines & standards, etc. Should have good communication, organizational and people skills, computer knowledge. Performs other duties as assigned by RN and DON. Experience in Home Health Care is required, at least one year.
