SKILLS: Basic computer skills and data entry skills; excellent communication skills. ABILITY TO: Handle multiple priorities, build positive relationships, maintain a positive attitude and a warm and friendly demeanor; efficiently work in a team environment; travel as required for assigned site(s) and designated meetings/trainings. • Physical Demands • Lift and/or move up to 50 pounds and push 100 pounds on wheels. Specific vision abilities include close vision and the ability to adjust focus. High manual dexterity. Reach above shoulder level, bend, squat, sit, stand, stoop, crouch, reach, kneel, twist/turn, and good finger sensation. Hearing adequate for phone work and clear speech for staff and patient communication. • Lifetime clearance of background check and fingerprinting through the CDSS • California driver’s license with a good driver’s record and vehicle insurance • Full time car availability • Medical release form signed by primary physician or DCI Medical Director • Must submit TB clearance • Must Complete Statement of Prior Criminal Convictions • No allergies to disinfectants (alcohol, surfactants, iodine, bleach, or common cleansers) • Agreement to comply with Diamond Care Inc. Safety Protocols • Good Record Keeping • Able to follow Physician Orders for Life-Sustaining Treatment • Able to maintain an on-call status on either an intermittent or regularly scheduled basis • Must have access to emails/text messages to be checked daily • Adhere to state and federal compliance guidelines related to HIPPA Job Functions The following describes the primary responsibilities expected of Care Providers employed by Diamond Care Inc. Various additional duties may be required when assisting a bedridden Client. • Demonstrates effective communication and problem- skills. • Works within a team care environment to provide excellent patient care services within scope of practice as established by the Medical Board of California. • Coordinates duties with Quality Assurance Manager, Office Administrator and from licensed providers, RNs, LVNs, Medical Director, General Manager, DCI Leads and trainers. • Pre-visit planning in collaboration with patient/responsible parties • Creating opportunities for patient access • Communicating effectively with care team throughout the day • Maintaining efficient patient care flow throughout the day • Providing continuity of care for the patient within the care team for that day • Collect and record patient data for vital signs, height, weight, and basic medical history • Medication administration • Specimen collection • Non-invasive treatments (e.g. ear lavage) • Removal of splints and orthotics • Application of prefabricated orthopedic appliances • Assisting in ambulation and transfers • Patient preparation for procedures and assisting provider in such • Provide patient instructions and education upon authorization of the provider • Emergency first aid and CPR • Nail cutting in healthy patients • Fit prescription lenses or use any optical device in connection with ocular exercises, visual training, vision training, or orthoptics • Records performance of authorized patient treatment and care in the medical record, consistent with DCI protocols • Participates in staff education and training activities in collaboration with office training staff. Maintains required competencies as established. • Assists in facility management including keeping patient areas clean and safe • Assists in responding to and resolving patient complaints as needed. • Proactively assumes responsibility for continuous improvement activities and is receptive to accepting other job duties as assigned and as delegated.
