Home Health Aide (HHA) Job Description
Hospice of the East Bay (HEB) is a community-based nonprofit that provides the hospice and palliative care services that support and comfort people and their loved ones through the final stages of life. Must have an HHA License to be considered for the position. A car is provided for patient visits for all of our HHA's. This position will help patients in Hercules through Oakland. POSITION SUMMARY: Reporting to the HEB Team Manager, this position is responsible for carrying out the duties assigned in order to increase the patient’s level of comfort and to maintain personal hygiene and a safe healthy environment for the patient. Although the Home Health Aide reports to a primary manager, HEB expects the Home Health Aide may work in more than one program or department, and assignments are given to best meet the needs of our patients. ESSENTIAL FUNCTIONS: Responsibilities of the Home Health Aide include, but are not limited to, the following: Performing personal care needs after given a patient’s assignment and attending to his/her requests. If unable to perform a certain task, communicates with Case Manager. Providing patients and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails). Following the plan of care as written by the RN. Demonstrating clinical care as defined in the Home Health Aide scope of practice. Assessing and reporting on the patient’s condition and significant changes to the RN. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. Recognizing and reporting emergencies. Keeping the patient’s living area clean and orderly, as assigned. Adhering to the organization’s documentation and care procedures and standards of personal and professional conduct. Participating in quality assessment performance improvement teams and activities. Participating in IDT meetings. Respectful of patient and family/caregiver environment and patient’s personal needs. Maintaining inventory of disposable supplies. Maintaining cleanliness and infection control standards. Performing other duties as assigned. POSITION REQUIREMENTS: Must be a compassionate team player who displays confidence and the ability to work independently with little or no supervision. Must demonstrate initiative, dependability and good judgment. Must be able to work collaboratively as a team with strong communication skills and be flexible with work assignments and professional when dealing with clients and staff. Must adhere to department goals, objectives and performance standards. Able to establish and maintain cooperative relationships with internal and external customers. Able to work well under pressure and handle multiple, changing and competing deadlines. Must be able to understand and carry out directions and instructions and maintain patient documents. Must be physically able to perform duties as assigned and have reliable transportation. EXPERIENCE AND EDUCATION: Must complete the Home Health Aide competency test with a passing score of 75% or better. Must have completed an official Home Health Aide training course and have a Home Health Aide Certificate issued by the State Department of Public Health. If there has been a 24-month lapse in furnishing services, the individual must complete another state approved Home Health Aide program before providing services. Valid CPR certification.
