Hospice experience and bilingual/Spanish speaking is preferred.
Responsibilities include but are not limited to: conducting patient visits and completing tasks as ordered in the patient care plan followed by timely EMR (electronic/computerized medical record) documentation, reporting to and communicating with interdisciplinary team (and facility staff if indicated), maintaining current licenses and medical clearance, attending mandatory training and meetings as required.
Requirements:
Current CA CHHA license
Current CPR certification
Current medical clearance to work (TB, Influenza, physical exam, etc.)
Valid CA driver's license, vehicle and auto insurance
