Home Health Aide (HHA) Job Description
Home health aide services may also be provided by a CNA subject to the same supervision described below. If necessary, they may also be provided by an LVN or RN. DUTIES: Provides personal care and related services in the home, under the direction, instruction, and supervision of the staff nurse and the Director of Patient Care Services. Tasks to be performed by an HHA must be assigned by and performed by the HHA. Under no circumstances may an HHA be assigned to receive or reduce any intravenous procedures, or any other sterile or invasive procedures, other than rectal temperatures. Responsibilities may include but are not all included to: 1. Follow the patient care plan of the patient 2. Report any changes to the leading nurse 3. Plan and prepare nutritious meals 4. Assist the client with ambulation as approved 5. Encourage the client to become as independent as possible according to the care plan 6. Maintaining an open communication process with the patient, representative, caregivers, and family 7. Gives simple emotional and psychological support to the client and other members of the family 8. Honors the patient's rights 9. Maintain hygiene of the patient 10. Complies with infection control 11. Work with personnel of other community 12. Attend-in service as required 13. Confer with the Supervisor about the client's status and care, and participate in the planning of service. 14. Works within the Home Aide's scope of practice, as directed by a Registered Nurse or other appropriate skilled professional and reports in writing to the Supervising Note.
