On Call Rotation LPN

Do you have a heart for hospice? Pinnacle Hospice is currently seeking an experienced LPN to join our team! This position is primarily responsible for seeing hospice patients in their place of residence, providing hospice and palliative care services after hours and/or on weekends. This position is back up to our triage team, and is actively on call , performing routine services to meet our patient’s needs. Part time position immediately available with potential for full time salary position. Starting rotation would be Monday-Friday 430pm-8am Saturday/Sunday 8am-8am, 7 days on/7 days off. Per diem rate daily for on call availability and hourly rate for travel and time worked. Hospice experience required. Pay rates discussed upon interview and based upon experience. Job Type: Part-time
