Personal Care Assistant (PCA) Job Description
Communicating effectively and efficiently with all Family Care staff to ensure that all issues and incidents are resolved in a timely manner. Implementing and following the participant’s Plan of Care Attending all mandatory staff meetings/trainings that are scheduled during employment Maintaining weekly service logs and detailed progress notes Working rotating schedules and being on-call, as needed Maintaining all participant information in a confidential manner Clocking in and out on time (via EVV system)for an assignment and remaining active the full shift Reporting incidents and suspected abuse and/or neglect immediately Maintaining each participant’s home book with proper paperwork, i.e.: service logs, timesheets, progress notes, and copies of all service logs Submitting weekly timesheets/ service logs with all applicable signatures and without errors by the agency dead line each week. At least a year work experience. Working with the elderly and physically and mentally disabled population. High school diploma or GED equivalent.
