Job description
The Wound Care Coordinator reports to the Director of Nursing and is responsible for delivering quality wound care and preventative measures to maintain skin integrity to assigned residents in compliance with applicable federal and state guidelines and established policies and procedures. Strong customer service and interpersonal skills required to interact with residents, families, physicians and other members of the multidisciplinary team. Must also possess the ability to pass medication and provide technical nursing care, recognize significant signs and symptoms of disease.
Responsibilities:
Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation and evaluation of residents wound and skin integrity
Collecting and documentation of residents’ skin status including medical history.
Wound care prevention and prevention through plan of care
Sets priorities for nursing action for wound care and prevention
Communicates with physicians regarding resident wound needs
Initiates nutritional interventions to ensure that residents with wounds are provided nutrition to promote wound healing.
Performs direct resident care utilizing established procedure, policies, and standards
Administer treatments and procedures in accordance with the physician’s orders
Maintains the comfort, privacy, and dignity of each residents while performing treatments
Communicates and interacts effectively, courteously, and tactfully with residents, visitors, family members, peers, and supervisors
Conducts rounds frequently to ensure that residents with incontinence are receiving pericare and preventative ointments timely. Discipline non-compliant staff accordingly with consultation from the DON
Performs duties in a safe manner in accordance with established policies and procedures
Performs all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves and proper disposal of soiled material
Requirements:
RN or LPN license issued by the State of Illinois
CPR Certified
2+ year Wound Care Nursing experience
Wound Care Certification preferred but NOT required
Excellent assessment skills
