Licensed Practical Nurse (LPN) Job Description
LICENSED PRACTICAL NURSE Job Summary: To assist Case Managing RN in providing high-quality nursing care to agency clients, with coordination of services with other team members Reports To: Clinical Supervisor or Branch President Responsibilities: 1. Is compliant with all policies, procedures, and regulations set by the Federal and State governments, the agency, and other regulatory agencies, i.e. OSHA, Worker’s Compensation, State licensure, and labor laws. 2. Coordinates client care with Case Managing RN and Clinical Supervisor. 3. Provides nursing care to assigned clients. 4. Participates in in-services, meetings, and case conferences. 5. Follows plan of care re client care and client and family instruction; recommends changes to plan of care based on clinical findings. 6. Notifies physician, Clinical Supervisor, and Case Managing RN of changes in client condition. 7. Completes and submits all appropriate clinical documentation per agency policy. 8. Makes appropriate referrals to community resources. 9. Monitors client lab results. 10. Maintains strict confidentiality of information relating to clients, employees, and the agency. 11. Promotes customer satisfaction for all clients, employees, and visitors to the agency. 12. Follows the agency’s Code of Ethics and Clients Rights. 13. Approaches tasks with a positive and flexible attitude and practices team behaviors. 14. Participates in after-hours, holiday, and weekend “on-call” rotations as assigned. 15. Performs other duties as assigned by the Clinical Supervisor or Branch President. 16. Maintains continuing education as required by state licensure standards and provides documentation of same. Job Requirements: 1. Knowledge of federal, state, and local regulations and agency policies and procedures. 2. Competent in the provision of nursing care within the scope of the LPN license. 3. LPN degree from an accredited school and licensure in Colorado; 1 yr clinical experience preferred. 4. Current CPR training/card. 5. Current professional liability insurance. 6. Able to use a computer, telephone and voice mail, fax machine. 7. Able to use blood pressure cuff, stethoscope, pulse oximeter, personal protective equipment, venipuncture equipment, and other tools and equipment as necessary. 8. Able to demonstrate good written and oral communication skills, including writing or typing notes legibly. 9. Able to travel to clients’ homes and to work in unpredictable home environments. 10. Able to lift 50 lbs unassisted and walk or navigate. 11. Able to hear and observe the client during treatment. 12. Able to effectively teach clients and family. 13. Able to demonstrate effective time management skills.
