Licensed Practical Nurse (LPN) Job Description
Position Summary: The LPN/Charge Nurse is responsible for the complete care and safety of all residents on the unit. Qualifications: LPN currently licensed by the state of Ohio Two (2) years nursing experience preferred but not required Six (6) months of Geriatric nursing experience preferred but not required Must be self-motivated and willing to engage in not only hands-on care, but to follow-up with documentation compliance. Must posses CPR and First Aid certificates or willing to obtain each within six (6) months of employment. Essential Duties and Responsibilities: Administers medications being careful to follow all physician orders, policies/procedures, and regulations. Completes accurate, complete, timely, legible documentation including medication administration records and treatment records. Coordinates resident care under the direction of the resident’s physician including calling physicians, implementing physician orders, physician rounds, consults, other services in Concordia’s continuum of care, rewrites. Supervises the care provided on the unit including monitoring quality of care, monitoring completion/accuracy of Nurse Aide documentation, coaching, counseling, delegation, coverage for call-offs. Makes walking rounds and assists in direct resident care as necessary, including serving and feeding residents. Administers treatments, including, but not limited to: dressings, catherization, suctioning, oxygen, etc. Identifies/monitors significant changes in residents’ conditions, and proactively and professionally communicates changes to charge nurse/supervisor, the physician, and other members of the health care team to keep them aware of changes in the resident’s care plan, condition, incidents, needs, and problem resolution. Assists in admission and discharge procedures. Gives and receives shift reports. Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
