Licensed Practical Nurse (LPN) Job Description
Qualifications: • Must be a licensed RN or LPN, in good standing, in the state of Massachusetts. • At least one year experience in Level II long-term care, CPR certification. • Must be aware of DPH regulations governing such care. • Must always adhere to nursing code of ethics. Responsibilities: • Complies with all procedures and policies as developed by Director of Nursing Services. • Counts controlled drugs with off-going nurse. • Makes round of the unit with off-going nurse. • Takes report from off-going staff. • Makes out assignments for CNAs, including breaks and lunch. • Writes significant patient info into report book throughout day. • Maintains charts and reports in proper order. • Assesses, administers medications and treatments on short term patients and/or long term residents as assigned. • Documents via nurse’s notes at least weekly on long term care residents and every shift on Medicare A and managed care patients/residents. • Ensures care plans exist and are added to every shift on every assigned resident/patient. Records all changes in both the nurses’ notes and in care plans when appropriate, e.g., actual falls, behavior issues, pressure ulcers, etc. with actual plans for interventions and treatments. • Completes all assigned monthly summaries in a timely and accurate manner. Understand that all summaries MUST agree with CNA Care Tracker sheets. Make sure, during the month in question, that CNA Care Tracker sheets are being done and done correctly every shift. Indicate corrections to the documentation as needed ensuring that the highest level of care being provided is being recorded. • Provide guidance and support, including helping with difficult transfers, lending a helping hand when CNAs are working “short” including completing physical tasks with residents as needed, e.g., feeding, bathing, toileting, etc. • The expectation is that nurse on cart will lock cart and answer residents’ call lights in lieu of letting them ring so long as to be an extreme falls risk. Lights are to be answered immediately. • Responsible for CNAs completion of assignment and paperwork. Check that AFOs, TEDs, ACE wraps, etc. are applied correctly. Check that residents are turned every two hours and changed every four hours and as needed and that I &Os are filled out as well as BM books and mealtime record books. Assure that weights are done and recorded without any missing weights. • Check the toileting and ambulation lists and assure the CNAs are doing them as well as any exercises the PT assigns them to do. The results of these are to be recorded by CNAs in appropriate binders. Check to make sure the CNAs are taking their residents out of Memory Loss Program to be toileted every 4 hours and when needed. • Ensure the highest possible quality of care at all times. • Complete an incident report and include a fall follow up sheet when indicated. Transfer the occurrence, resident goal, and plan to prevent further unwanted occurrences in the care plan, and record incident in nurses notes and shift report. Assure that MD and residents’ families are always notified of any untoward event. With falls, please complete a neuro sheet and record vitals and nurse’s notes for 72 hours. • Complete all admissions paperwork within 24 hours. Pass on any unfinished MD orders whether or not on an admission to next shift after a good-faith attempt to finish all in a timely manner. Nurse’s notes every shift for the first 72 hours. • Be respectful, courteous, professional and supportive of your supervisors as well as those you supervise. Using the progressive discipline system, please take notes on anything that you notice that could potentially harm a resident, either physically or emotionally. Report incidents to Unit Manager and/or Director of Nursing Services immediately. • Make sure that all patient equipment is clean and functioning correctly every shift. • Be a family liaison and problem solver. • Rather than doing just 24-hour order
