Licensed Practical Nurse (LPN) Job Description
* Provide direct patient care, to include using a variety of skills and necessary coordination of services
* Monitor vital signs and complete appropriate documentation as required by policy and procedure and regulation.
* Follow the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Nurse Supervisor.
* Assess patient needs, plan for, evaluate and modify care to meet goals of skilled nursing interventions.
* Coordinate homecare services with the Nurse Supervisor,
* Accept and transcribe physician’s orders as directed.
* Forward all clinical, progress, and summary notes to the Nurse Supervisor for coordination.
* Attend and participate in patient care conferences.
Job Requirements:
* Successful completion of nursing education from an approved school of nursing.
* Current North Carolina RN/LPN licensure and one year of clinical nursing experience.
* Ability to independently organize work and provide effective health care services to homebound patients.
* Ability to ensure appropriate patient care as supervised by the home health Nurse Supervisor, and document thoroughly, accurately, and completely on a timely basis.
* Effective communication skills (oral and written).
* Current CPR and First Aid certification.
* Must be willing to provide in home nursing care to infants, children, and adolescents
* Ability to work independently and with minimal supervision
Responsibilities:
* One to one patient care in a home environment
* Execution of physicians prescribed plan of care and compliant documentation of care provided in system of record
* Administration of prescribed medication, treatments and therapies
* Patient assessments, coordination of care
* Health promotion, teaching, and training of family members
