Licensed Practical Nurse (LPN) Job Description
JOB SUMMARY: Reports directly to the Director of Nursing and oversees, assists and directs with the hands on care provided by staff in support of residents activities of daily living (ADL’s). Serves as charge nurse for neighborhoods as designated and when assigned, in charge on the premises in the absence of the DON, ADON and Executive Director. QUALIFICATIONS: Current minimum knowledge, skills and abilities, experience, or educational or professional qualifications required for licensed GA Assisted Living Facilities that care for adults with serious cognitive impairments. The person holding this position must be at least an LPN, licensed by the State of Georgia. 1. Helps in the implementation in on-going individualized service plans to meet the assessed needs of each resident; 2. Communicates and coordinates with physicians in the absence of the ADON, DON, any new orders, discontinued orders or changes in orders, keeping ADON, DON apprised of the changes; 3. Assist with coordination of therapies, including PT, OT Speech Therapy and ensures that the direct care staff is aware of the plans of care and goals; 4. Documents in nurses notes daily, transcribes orders, follows through with the Pharmacy protocols, monitors monthly EMARS; 5. Monitors safe practices of direct care staff; 6. Ensures that the activities are performed as directed by the Activity Director with the direct care staff; 7. Oversees medication passes; 8. Assists with personal care of residents when needed; 9. Interacts with family members in the absence of the ED, DON and/or ADON; 10. Reviews ADL sheets and notes of CNA,RA/Med Aids; 11. Keeps DON and /or ADON abreast of all issues, problems, changes, etc. of residents and/or families;
