Hospice RN Case Manager

Golden Rule Hospice is looking for Full-Time Registered Nurses to manage patients in the following areas: Hall County and Cherokee County. One year of hospice or home health experience, preferable. Minimum 2 year’s experience as a Registered Nurse. Reliable transportation with appropriate insurance coverage Good communication skills (verbal and written) REGISTERED NURSE RESPONSIBILITIES: Regularly assesses and reassesses the nursing needs of the Hospice patient. Provides Hospice nursing services, treatments, and preventive procedures. Initiates nursing procedures appropriate for the patient’s Hospice care and safety. Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition. Teaches, supervises, and counsels the Hospice patient and family members about providing care for the patient. Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care. Attends team conferences. Works with the marketing team to educate the community about hospice services.
