Position: Licensed Practical Nurse (LPN) Company: True Care Home Health Schedule: PRN / Part-Time / Full-Time (Flexible) Location: West Central Florida Overview: True Care Home Health is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our dedicated team. Our LPNs provide high-quality in-home care that supports the independence, dignity, and well-being of our senior and adult clients. Key Responsibilities: Provide skilled nursing care under the supervision of an RN or clinical manager Perform wound care, medication administration, and treatments per care plan Monitor and document patient conditions and communicate changes promptly Assist with chronic disease management and patient education Collaborate with caregivers, families, and multidisciplinary teams Maintain accurate clinical documentation according to agency standards Deliver care with professionalism, empathy, and respect Qualifications: Current Florida LPN license Home health experience preferred, but not required CPR certification (current) Strong communication and organizational skills Reliable transportation
