Licensed Practical Nurse (LPN) Job Description
Duties and Responsibilities include the following. Other duties may be assigned.
§ Performs licensed nurse tasks within scope of practice as allowed by state law and provides
direct assistance with activities of daily living and support to residents.
§ Practices positive resident relations by introducing himself or herself to new residents upon
move-in and by responding promptly and positively to requests, complaints and emergencies.
Converses with residents to promote social interaction and provide emotional support.
§ Is visible and available to residents and aware of their location.
§ Monitors or assists with resident admission and discharge processes to ensure timely
completion of required documentation and compliance with established policies and
procedures.
§ Assists with/develops preliminary and comprehensive evaluation (Resident Functional
Evaluation) and written care service for each resident, identifying service needs and goals to
be accomplished. Communicates service plans and/or changes in services to appropriate
personnel.
§ Completes, maintains, follows-up on required documentation, forms, reports, evaluations,
results, etc. as appropriate.
§ Assists with coordination of/coordinates new resident orientation process and assists with
resident move-ins.
§ Ensures accurate recordkeeping and proper maintenance of resident records by reviewing
chart documents regularly. Properly records and transcribes physician orders in resident
charts.
§ Conducts daily visits with residents; observes, evaluates, documents and reports any changes
in resident’s condition.
§ Performs, documents, evaluates effectiveness of prescribed treatments as directed.
§ Consults with physicians in providing care, treatment, diagnostic and therapeutic services and
notifies physician of any resident accidents/incidents and changes in condition as directed.
§ Provide oral/written reports to Resident Care Director (RCD)/Director of Nursing
(DON)/Executive Director (ED) concerning status of residents, including problem areas.
§ Functions as liaison to and communicates effectively and regularly with residents’ health care
providers/physicians, pharmacists, medical personnel and family members.
§ May informs family of any changes in resident’s health as they occur and as directed by
RCD/DON/ED
§ Assists with coordination of medication, hospitalization and/or physician visits as directed.
§ Assists with/manages inventory, storage and purchase of medical supplies to ensure
availability of adequate levels of medications, as well as supplies and equipment.
§ Assists with supervision of storage, distribution and discontinuation of medication, including
proper record-keeping. Secures and maintains department supplies and equipment.

§ Assists with administering medications and/or monitors self-administration of prescribed and
over-the counter medication in accordance with medication guidelines and ensures daily
maintenance of medication observation record (MOR or MAR) for each resident.
§ Reviews medication charts for accuracy, completeness and adherence to physician orders.
Notifies Resident Care Director/DON or Executive Director of drug/narcotic discrepancies
discovered during shift.
§ Completes written reports and routine resident record reviews and updates as scheduled.
§ Documents and reports incidents, unusual occurrences, and observations of changes in
resident health or behavior timely and accurately.
§ Maintains confidentiality regarding residents’ personal and health information and respect
residents’ privacy in accordance with HIPAA regulations. Understands and adheres to the
resident bill of rights as defined by State law.
§ Demonstrates exemplary work ethics through integrity, compassion and professional business
practices.
§ Provides hands-on support to resident care staff during his/her shift; ensures provision of
quality resident care and compliance with regulatory guidelines and established standards.
