Licensed Practical Nurse (LPN) Job Description
Position Overview
The Licensed Practical Nurse (LPN) provides high‑quality, compassionate care to clients with complex medical needs, including cancer diagnoses, feeding tubes, and tracheostomies. This role requires strong clinical skills, excellent communication, and strict adherence to safety and infection‑control standards. The LPN supports patient comfort, monitors health changes, and ensures all care is delivered according to state regulations and agency policies.
Key Responsibilities
Patient Care & Monitoring
Provide direct nursing care to patients with cancer, including symptom monitoring, comfort care, and support during treatment cycles.
Manage and monitor feeding tubes (G‑tube, PEG, J‑tube) including:
Safe administration of feedings and medications
Site care and infection prevention
Troubleshooting common issues (leaks, clogs, discomfort)
Provide skilled care for tracheostomy patients, including:
Suctioning
Cleaning and maintaining the trach site
Monitoring respiratory status
Emergency response for airway concerns
Observe, document, and report any changes in patient condition to supervising RN or provider.
Clinical Procedures & Compliance
Administer medications safely and accurately.
Follow all care plans, physician orders, and agency protocols.
Maintain accurate and timely documentation in Alora or designated EMR.
Ensure infection‑control procedures are followed at all times.
Maintain required location tracking and communication standards during all shifts.
Communication & Coordination
Communicate clearly with patients, families, caregivers, and the clinical team.
Provide education to patients and families regarding feeding tube care, trach care, and symptom management.
Collaborate with interdisciplinary teams to ensure continuity of care.
Respond promptly to calls, messages, and updates from the agency.
Safety & Emergency Response
Recognize and respond to medical emergencies quickly and appropriately.
Maintain readiness to manage airway complications, feeding tube issues, or sudden changes in patient condition.
Ensure patient environments remain safe, clean, and supportive.
Qualifications
Required
Active Licensed Practical Nurse (LPN) license in the state of North Carolina.
Current CPR certification.
Experience working with:
Cancer patients
Feeding tubes
Tracheostomies
Strong clinical judgment and ability to work independently.
Reliable transportation and ability to travel to patient homes.
Preferred
Experience in home health or hospice.
Familiarity with Alora or similar electronic charting systems.
