Licensed Practical Nurse (LPN) Job Description
Our House of Portland is a small, nonprofit RCF located in SE Portland. We care for individuals living with HIV who may be dealing with other comorbidities such as substance use disorder, kidney disease and heart failure. We also care for individuals who are on palliative or hospice care. Our House is an inclusive learning environment with an interdisciplinary team of Nurses, Social workers, Peer Recovery Mentors, and Caregivers who work together to meet the needs of our complex population. This is a rewarding position for someone who is looking to work in a nonjudgmental environment of community and healing. We are hiring for a night shift LPN or RN to join our team. The Licensed Practical Nurse (LPN) provides direct care to the residents as well as environmental care of the facility with the caregiver on duty. The Licensed staff performs the primary function by assisting in assessing, planning, implementing, and evaluating the care of our residents. QUALIFICATIONS: Current license in good standing to practice as an Licensed nurse in the state of Oregon. CPR certified. One year of nursing experience preferred. COVID Vaccinated RESPONSIBLITIES: Perform nursing procedures in accordance with licensure and skill level, utilizing principles of safety and infection control. Provide assistance with activities of daily living. Administer and document medications and treatments using electronic medication administration record. Contribute to the evaluation of the resident for appropriateness of care, medications, and treatments, as well as their response to the service plan. Participate in shift planning with team immediately after report. Attend nursing staff meetings and in-services. Respect and maintain privacy, confidentiality, and appropriate boundaries of residents and staff. Responsible for maintaining professional standards for licensure. Must be able to lift 30-50 lbs., push 21-50 lbs., carry 11-20 lbs., pull 21-50 lbs. on an occasional basis. The job also requires occasional bending, crouching, grasping, squeezing and the use of foot controls as well as stair climbing and repetitive use of wrists, arms, hands, and shoulders. More frequent job requirements include twisting, walking and reaching above the shoulder.
