LPN Community Nurse Coordinator

The LPN shall: 1. Provide nursing care only under the supervision of the Lead LPN, as defined by the Nurse Practice Act, and the State Board of Nursing. 2. Possess knowledge of and adhere to federal, state and DDSN regulations pertaining to medical/nursing care of consumers served. 3. Provide nursing services following established policies and procedures of the agency. 4. Be accountable for his/her nursing practice and is responsible for meeting the established standards of his/her profession and for practicing within the scope of licensure. 5. Assist the Lead LPN in care and management of the agency’s nursing needs. 6. Assess, train and indirectly supervise unlicensed personnel (ULP) as needed regarding medical care of consumers served. 7. Promote and evaluate the quality medical care of agency consumers served. 8. Maintain medical records in a concise manner, according to agency policies. 9. Insure that appropriate nursing care is provided to all consumers as prescribed by the physician. This includes but is not limited to the following: administering medication, performing treatments, and any other medical need that may arise. 10. Responds appropriately to all medical emergencies of consumers in a timely manner when on campus. 11. Insure that Infection Control is maintained at all times within the home as per policy and Infection Control Standards. 12. Insure that all activities of daily living are met as specified in the consumer’s plan of care and/or according to supervisor request. Assists direct care staff in facilitating ADLs as needed. 13. Insure consumer’s diets are followed as ordered and according to dietary policy & manual. 14. Insure that accountability of consumers is maintained at all times. 15. Perform and/or assist with transferring and/or relocation of consumers. 16. Document and complete all assigned paperwork MINIMUM TRAINING AND EXPERIENCE: 1. Completion of an accredited School of Nursing 2. Current LPN Licensure with the South Carolina State Board of Nursing. 3. Minimum (1) year of nursing experience required. Or recent graduation of nursing program Hours are primarily first shift with on call rotation for evenings/weekends/holidays.
