Licensed Practical Nurse (LPN) Job Description
LPN Home Health Quality Assurance (QA)
Serenity Home Health Care Inc. is seeking an experienced Home Health Clinical Manager to supervise Home Health Services in accordance with the Physician's Plan of Care. The QA nurse Participates in activities relevant to the direct provision of home health services and is responsible for compliance with CHAP and IPI policies and procedures, federal and state regulations, IDPA, IDPH, Medicare, payer, and accrediting rules, regulations, and standards.
Responsibilities
- Responsible for the company’s Quality Assessment Performance Improvement (QAPI) program. Assists with the overall development and implementation of the QAPI plan, and the identification of goals related to patient outcomes and coordinating, participating, and reporting of activities and outcomes results.
Responsible for the review of patient medical records for compliance with federal, state, and local laws, accrediting organization standards (if applicable), and company policies and guidelines.
Develops a cooperative relationship and communicates effectively and professionally with physicians and referral sources.
Assists Nursing Supervisor in conducting patient care conferences, staff meetings and maintains documentation.
Participates in community education projects.
Responsible for the orientation of new patient care personnel.
Assists with staffing visit assignments and on-call.
Perform other job-related duties as assigned.
Qualifications
Current LPN license.
Two (2) years of home health nursing experience.
At least two (2) years of healthcare experience in the field of supervision including one year of supervisory experience in-home care or a closely related service.
Graduate of an approved school of professional nursing and currently licensed in Illinois.
Ability to exercise initiative and independent judgment.
Ability to work with individuals to enlist the cooperation of many people to achieve a common goal.
Must be proficient in Axxess EMR Software
