Registered Nurse (RN) Job Description
We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our healthcare team. The ideal candidate will deliver exceptional patient care while demonstrating expertise in anatomy, pain management, and effective communication across diverse patient demographics. This role requires collaboration with healthcare professionals to achieve optimal patient outcomes, including support in family planning initiatives as needed. The RN will provide skilled nursing care to patients in their homes, following physician orders and individualized care plans under the supervision of the Director of Clinical Services or Nursing Supervisor. Responsibilities Patient Assessment and Care -Assess and monitor patients' health, including vital signs and symptoms. -Perform initial and ongoing patient evaluations, updating care plans as required. -Deliver nursing services in accordance with care plans and physician orders. Treatment and Documentation -Administer medications and treatments as prescribed. -Implement pain management strategies tailored to individual patient needs. -Utilize eClinicalWorks or other electronic health record systems for accurate documentation. -Document clinical notes and care summaries promptly. Patient and Family Education -Educate patients and families on treatment plans, health management, and preventive care. -Provide resources and guidance during family planning discussions. Coordination and Communication -Collaborate with interdisciplinary teams to develop comprehensive care plans. -Communicate changes in patient conditions to the healthcare team. -Coordinate patient services and facilitate referrals to therapists or social workers as needed. Regulatory Compliance and Leadership -Maintain a safe, sterile environment in compliance with health regulations. -Supervise and educate nursing staff, conducting care conferences for assigned patients. -Participate in peer reviews, in-service programs, and performance improvement initiatives. Additional Duties -Provide total patient care and critical care support when necessary. -Complete OASIS assessments and ensure accurate ICD-9 coding. -Participate in on-call duties as assigned. Requirements Licensure and Experience -Valid Registered Nurse (RN) license in the state of practice. -Preferred: Two years of nursing experience. -Experience in critical care or emergency settings is advantageous. -Familiarity with long-term care or medical office environments is beneficial. Skills and Knowledge -Strong understanding of anatomy, physiology, and pain management. -Proficiency in electronic health record systems (e.g., eClinicalWorks). -Excellent communication and teamwork abilities. -Compassionate approach to patient care with a focus on enhancing quality of life. -Familiarity with botulinum toxin treatments is a plus. Working Environment -Works indoors at the agency office or patient homes, with required travel.
