Licensed Vocational Nurse (LVN) Job Description
The QAPI Coordinator has the overall responsibilities for the development and implementation of the QAPI program. He/she reviews hospice medical records for assessment of the quality of clinical services to ensure adherence to all policies and procedures and applicable standards and makes recommendations for changes in policies, procedures, systems and for staff education. REPORTS TO: Administrator SUPERVISES: Medical Records, if applicable QUALIFICATIONS: • Education: Graduate of an accredited school of nursing or school of vocational/practical nursing. • Credentials: Current license within the State as an RN or LVN/LPN. • Experience: At least two years of varied work experience as a licensed nurse and preferably one of the two years as QAPI nurse. • Core Competencies: Ability to make meaningful observations and write comprehensive accurate reports. Knowledge of QAPI functions/Medicare COPs, State regulations. Good computer skills. Ability to be attentive to detail. • Possesses excellent verbal and written skills. • Possesses a sympathetic attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team. Other: Valid driver's license, and auto insurance. FUNCTIONS & RESPONSIBILITIES: 1. Works with the Administrator to provide an on-going, comprehensive assessment of the quality of care and services provided. 2. Coordinates the implementation of the program’s Quality Assessment and Performance Improvement Plan to assess the quality and appropriateness of patient care. 3. Coordinates all QAPI audits and studies as indicated in the plan. 4. Adheres to the highest standards of professional and personal conduct in the performance of his/her duties. 5. Develops and implements problem focused audits. 6. Reviews all incidents, concerns and grievances to ensure appropriate follow-¬up and documentation have occurred. 7. Identifies patterns and trends and reports them to the Administrator and QAPI committee. 8. Conducts quarterly QAPI meetings, reports findings and coordinates development of action plans. 9. Completes monthly reports to the Administrator. 10. Utilizes problem-solving techniques to address areas of non-compliance. 11. Maintains current information and requirements related to applicable laws, regulations and standards. 12. Assures the program is reviewed at least annually. Included is an evaluation of all services that were provided and patients/caregivers response to those services. Attention is given to: symptom management, pain control, stress management, continuity of care and inpatient care. 13. Maintains continuing education through academic studies, seminars, and workshops 14. Attends and participates in in-services, staff meetings and case conferences. 15. Completes all documentation in an accurate and legible manner. 16.
